Question:   RE Section VII. B. 2. Data collection and Survey Follow-up Will you be creating a unique web address/survey links for each of the 15,000 business establishments in the sample? I am aware that Qualtrics has this capacity, it it should help with tracking responses.

Answer:   Our plan is for each establishment to have a unique ID for tracking responses, but a single web address.

Also include contracts approved for ODJFS or institutions of higher education:” Do you mean July 01, 2010?

Answer:   Yes, July 1, 2010.

Question:   Hello, Could you please tell me: 1) What sort of data file will be produced when respondents complete the survey from ODJFSs website and 2) If ODJFS could please make available the attachments from the RLB in Word format. Thank you.

Answer:   1)The online questionnaire will be conducted using Qualtrics software. Data from that program can be output in CSV and SPSS (.sav) formats. If a vendor chooses to use their own CATI or data entry software rather than using the online option, the output should be in a .cvs, .xls, or .sav format. ODJFS will supply the Qualtrics file structure to the vendor so it can match it in their output file(s). 2)Due to our current web posting technology, ODJFS is unable to post Word formatted documents to procurement opportunities. To request all Word formatted documentation associated with this procurement opportunity please forward a email to Anthony Norwood at anthony.norwood@jfs.ohio.gov and include you name, phone number, and current email address. ODJFS will only responds to requests during the published Q&A Period of 8/17/2010 to 8/25/2010.

Question:   This proposal appears to be very similar to a proposal released just a few weeks ago. Is this an additional requirement to the previous RFP?

Answer:   This RLB is similar to the previous RLB. As noted in the posting letter, it is now an open market opportunity. Also, a few requirements were relaxed or reduced based on Indiana's experience conducting their green jobs survey.

Question:   On page 2 of the RLB under II. RE: Time and Date of Submission & III. Anticipated Procurement and Project Timeline Under section II date for submission of bid is September 1, Under Section III the deadline to submit proposals is September 7. Please advise which is correct. Thanks!

Answer:   The correct submission date is September 7, 2010 by 3:00 P.M.
