Question:   Re Section 3.1 Scope of Work and Specification of Deliverables: Section B. Distribution of Survey (pg 22) Regarding the statement “Vendors must, at minimum, ascertain the appropriate response rate of at least a 95% confidence interval” Is there a margin of error that you want obtained on the results associated with the 95% confidence level? Re Section 1.8 Time Frames & Funding Source(pg 23) The time frame for Phase II distribution of survey (four weeks) does not lend itself to multiple survey mailings. Sound survey methodology includes a second planned mailing to non-respondents after a waiting period for collection of surveys form the first mailing (see Dillman, mail and Internet Surveys, 2nd Edition, 2000). After the second mailing, additional time is required for return of those surveys. Will it be possible to adjust the project schedule to allow additional time for phase II? Re Section 5.2 Section C Cost Proposal (pg 19) Is there a cost limit associated with this RFP? If so, what is it?

Answer:   1) No. ODJFS would expect the vendor to report the sampling margin of error. The previous two surveys have had an error rate of 5.8% and 6.5%. 2) Phase II can be extended into Phase III if needed. However, Phase III must be completed as scheduled by Week 12 and Phase IV must be completed as scheduled by Week 15. 3) ODJFS does not provide information on any budget limits. The vendors are to assess their actual costs based on the work described for the project and submit their cost proposal accordingly.

Question:   Re: RFP Section III/3.1/A. Survey Development (pages 7-8 of 23). Questions: - Will a cover letter be sent with the survey? - Who develops the cover letter content - Ohio or the supplier?

Answer:   Yes, a cover letter will be sent with the survey. ODJFS will develop the content of the cover letter with assistance from the selected vendor. The selected vendor's assistance will be limited to reviewing the cover letter for accuracy concerning dates, process for survey submission and contact information.

Question:   Re: RFP Section III/3.1/B. Distribution of Survey (page 8 of 23). Question: If the State is providing a database for the survey mailings, please confirm what information is included – e.g., mailing addresses, e mail addresses, phone numbers, etc.

Answer:   Only mailing addresses will be included on the excel spreadsheet. No email or phone numbers will be included in this file.

(The reason we ask is, if they are actual labels, additional costs for data entry would be required, increasing the overall cost.)
